To participate in the EZ-D™ Incentive Program follow these easy steps:
- Either print and complete this form OR put the following information on an index card:
- name
- address
- e-mail address
- phone number
- choice of free EZ-D movie (select from any available title)
NOTE: Either a phone number or e-mail address is required just in case we need to contact you about your submission.
- Mail the form or index card in with the six expired EZ-D™ discs to:

- We will mail your free movie title to you within 2 or 3 weeks of receipt of your form or index card. If the submission is incorrect (i.e., if you send in less than six EZ-D™ discs, or you fail to complete all the information fields), we will contact you informing you of an error as long as the required phone number or e-mail address is provided.
